FEEDBACK : (Compliments, Comments, and Complaints Form)

Your comment and feedback are important to us at Chetana Hospital. It assists us to measure patients' level of satisfaction with our services and care provided, helps to plan future services and facilities as well as providing feedback to our staff.

If you have a complaint about the care or services provided, we encourage you to raise this immediately. Complaints can be raised with your contact nurse, nursing unit manager or the psychiatrist on duty.

All complaints will be dealt with in a strictly confidential manner, as soon as practicable. Responses will be relayed back to you either verbally or in writing. A form is available for patients to make a complaint, give an opinion, compliment or provide an improvement suggestion.
